All kinds of primary or secondary chronic kidney disease could cause kidney
damage. When the kidney disease progresses to certain degree where their kidney
functions have just 25 percent left, their kidney function would deteriorate
further to kidney failure, even if their basic disease has stopped activity.
With the kidneys' functions of filtering the toxins declining progressively,
the toxins would build up in the body. Long-term retention of toxins would pose
irreversible damage to other vital organs such as heart, lung, nerve system and
bones, bringing about catastrophic consequences.
According to scientific studies, the health of these organs are the most
important factors that influence the life span and life quality of the patients
on dialysis. That is, for kidney patients who have entered uremia or are even on
dialysis, it is crucial to lessen the damage to organs such as heart, lung. So
it makes great sense for kidney patients with uremia to receive renal
replacement treatment to clear the toxins produced in the body.
At present, renal replacement treatment includes hemodialysis, peritoneal
dialysis and kidney transplantation. Internationally, when the kidney patients
are at CKD stage 4, they should receive the relevant education about the
treatments, comprehend the pros and cons of each treatment method and prepare
for the treatment. When the patients reach CKD stage 5, the kidney physicians
would start the renal replacement treatment after evaluating all of the
patient’s conditions, including the primary disease, complications, uremia,
blood pressure, past medical history, age and other factors. In some cases, even
the patients’ GFR is above 15, their treatment should be started earlier.
Kidney transplantation
Kidney transplantation is a surgery where a “normal” kidney is transplanted
into the patient’s body to work for the patients, so it is the most effective
treatment. But this surgery is very demanding and before the transplant, kidney
patients would go through a series of tests to determine whether they are fit
for kidney transplantation or not. For example, if the kidney patients are
having active hepatitis B, TB, gastric ulcer, serious diabetes, they are not
recommended to do kidney transplantation.
Haemodialysis
Haemodialysis is widely used for patients with kidney failure, which includes
conventional hemodialysis, hemofiltration and HDF(hemodiafiltration). But
hemodialysis is not fit for patients with shock, severe cardiac insufficiency or
coronary heart disease, severe high blood pressure, severe hemorrhagic tendency
and cerebral hemorrhage.
Peritoneal dialysis
Peritoneal dialysis does not depend on machines, is easy to operate and
relatively cheap. Patients with following conditions should consider peritoneal
dialysis as their priority: 1.senior citizens. 2.patients with poor
cardiovascular functions. 3. patients who fistula is hard to build. 4.patients
who have abundant urine.
